COMMUNITY OUTREACH SERVICES

Older Adult Outreach and Support

 

Our team of Outreach Workers provides individual case management support to help older adults access the supports, services, and resources they need to age with dignity and in the right place. Our Outreach Workers can meet with older adults in their homes or at a convenient community location, where they will complete holistic assessments and work with them on identifying and setting practical goals to ensure their needs are met.

 

Services:

 

Our Outreach Workers can support with:

  • Information and referrals.

  • Assessment and goal-focused case management.

  • Assistance with forms for:

    • Pensions

    • Benefits

    • Transportation

    • Housing

  • Access and connection to volunteer programs and supportive services.

  • Navigation and connection to caregiver support.

  • Navigation and connection to elder abuse intervention services and support.

Calgary Seniors is one of the partner agencies that make up The Way In Network. The Way In is a collaboration of Calgary-based agencies dedicated to enhancing the quality of life and access to services and support for older adults living in the community and their families.
